
What You'll Need to Complete the Lease Agreement
- A copy of the title or last registration to verify ownership.
- A copy of the bill of sale or lease agreement to verify purchase date and price.
- A copy of the receipt for your Federal Highway Use Tax form 2290 Schedule I.
- A current drivers license and medical card.
- A copy of your and/or your driver's current Motor Vehicle Report.
- A $1000 security deposit and appropriate prorate fees.
- If you are a representative/driver for a future leasor, a Power of Attorney should be in your possession in order to sign all forms and documents. This document must be notarized and specify the area authorized.
- If you are coming from another carrier, you must make sure your lease is terminated with them and you have returned all plates and registration. Bring a letter from the carrier verifying termination.
- Complete the Owner/Operator application packet and lease agreement.
- Certified light weights. We must have three (3) separate certified documents:
- Certified light weight on truck only.
- Certified light weight on trailer only.
- Certified light empty weight on combined truck and trailer. This will be your working weight and should reflect all of the equipment on board and at least 1/4 tank of fuel.
- All light weights will be done in Yreka, California.
